Position Summary:Responsible for providing medically prescribed physical therapy to patients in the home in accordance with agency policies. Participates in developing and implementing the plan of care with goals and documentation appropriate to discipline guidelines. Provides supervision as needed to PTA per JCAHO/CHAP, Florida State and Federal practice act guidelines .Performs physical therapy procedures, evaluations, resumption of care, admissions, and diagnostic tests on patients for whom these have been medically prescribed and establishes measurable goals. Teaches appropriate treatment procedures and supportive activities to patients and family members.
Minimum Qualifications:- BS degree and graduation from an accredited school of Physical Therapy required.
- Work experience of at least one year as a Physical Therapist, Home Health experience preferred.
- Able to prioritize and manage time effectively and make independent decisions when necessary.
- Current Florida Physical Therapist License.
- CPR certification.
- Valid Florida or Alabama Driver's License and current Auto Liability Insurance
Main Duties & Responsibilities: - Performs physical therapy procedures, evaluations, resumption of care, admissions, and diagnostic tests on patients for whom these have been medically prescribed and establishes measurable goals.
- Teaches appropriate treatment procedures and supportive activities to patients and family members.
- Translates all exercises into functional activities or activities of daily living.
- Develops needed plan for modifying equipment, appliances, and the physical surroundings in patient's home.
- Interprets to the patient and family the implications of the treatment consistent with the actions and wishes of the physician.
- Helps the family accept responsibility for providing care.
- Provides families with information, support and encouragement to help motivate them in their progress.
- Helps individuals and families to accept and adjust positively to physical, mental, and social limitations and identifies barriers to desired outcomes.
- Evaluates client's progress towards goals on an ongoing basis, making changes in the therapy program as necessary, under the physician's direction.
- Teaches basic principles of good posture and body mechanics in relation to changing needs of individuals in all age groups.
- Maintains necessary records.
- Demonstrates rehabilitation procedures and supportive activities on selected patients to nurses responsible for follow-up services.
- Supervises home health aide if no other health care discipline is going to the patient's home.
- Participates in the recertification process and interdisciplinary team conferences.
- Completes OASIS assessments accurately at appropriate time points.
- Notes progress towards discharge planning.
- Communicates with physician and other disciplines regarding discharge planning.
- Participates in the agency PI process as evidenced by performing multidisciplinary chart audits, recording and reporting quality improvement data retrieved on a routine basis.
- Displays willingness to discuss weaknesses and make improvements in job performance.
- Demonstrates the ability to be flexible with workflow, department needs and work hours.
- Updates management regarding potential problems or concerns.
- Also responsible for consulting with clinicians or managers as needed.
- Maintains an understanding of the company's scope of services.
- Understands the infrastructure and how and where to transfer calls in the company.
- Maintains a professional image.
- Uses appropriate phone etiquette.
- Promotes a customer friendly atmosphere for all visitors and ensures patient confidentiality at all times.
